Death of Designated Beneficiary

In the event that you are pre-deceased by a designated beneficiary, you should notify the Fund office and request new designation of beneficiary forms. Designation of beneficiary forms for the 401(k) Fund, the Annuity Fund and the Health & Benefit Fund are also available on this website. The new designation of beneficiary forms are not effective until they have been submitted to the Fund office.
